Chris:

CHIRP needs you to download from the radio FIRST to determine which 
firmware the radio has installed,  THEN put in the repeater parameters 
into that file.   You may want to save that first downloaded file in 
case you need to start over.

The repeater settings you have look OK, By setting tone mode = TONE, 
the  tone squelch setting is ignored.  I've attached the CHIRP guide 
which explains all of the data fields.
