I thought there were Wikipedia pages devoted to the subject, and indeed there are, albeit they're a bit short. Note that it debunks the 'myth' that the captain has the final say in the matter...

As I recall, when I traveled to Vancouver a few years back and had my VX5R ham triband handheld with me, I had a couple of freqs programmed for the airport, including the tower. No one really questioned it very much. 73s Mike
