No penalty for dupes.   You  had 3
1 penalty for each call copied incorrectly.   You had 2, so  that's 4.
No penalty for busted exchange.   You had 1
1 penalty  for each not-in-log call.   You had 2, so that's 4.

Your total  QSOs removed should be 3+4+1+4 = 12, yet you had 11 removed.   
Perhaps  one of these problem exchanges had multiple errors (one contact with 
two  problems, like a dupe with a busted exchange).
