Center punch, then drill usually works, if the site is accessible.   Of 
course the rivet I need to remove always is the one that's not,  so the  
next best method I've found is a round ball-shaped grinding stone in the 
Dremel tool which allows you to get at it from even a shallow angle.   
It takes patience but you can grind away enough of the rivet that the 
remainder is easily pushed out.
