I think Doug is correct here.
4-wire = hot-hot-neutral-ground.
3-wire = hot-hot-ground.
The 4-wire is required if the appliance or other device
requires both 240V and 120V. The 120V is developed
across one hot and the neutral with the 240V being
developed across the two hots. If the device does not
require 120V, then a 3-wire configuration is used. A
ground is required in either case.
