My bad! I used the colloquial "Extra" rather than the correct full name.

That's right - it doesn't. But note that the elements have been renamed and 
post-2000 Element 4 is not exactly the same as old Element 4A

No, it wasn't!

The Advanced started as the old Class A in the early 1930s and its written 
test was at a level higher than the the General/Class B test, which was also 
used for the Conditional. The General written ("old" Element 3) was also used for 
the Tech until March 21, 1987, when it was split into two pars, 3A and 3B.

The written test for Extra (short name used for brevity) was created in 1951 
and was at a level even higher than the Advanced. The Extra was meant to 
replace the Advanced, and no new Advanceds were issued after December 31, 1952. 
However,  FCC changed its mind in the 1960s and reopened the Advanced to new 
issues in 1967.

When the Advanced was reopened, the Extra written test ("old" Element 4) was 
split into Elements 4A and 4B.
