Hi Dave,

It goes on the bottom of the board.  You can see it
in Appendix F Parts Placement Drawing, Bottom.  (Odd thing
here - the Acrobat Reader can't find it.)

Use the two outside holes of the three, i.e. the middle hole
is not used.  (The middle hole is grounded.)
