[ARC5] Screws

Gang,
 Anyone know where to get nickel plated brass phillister (?) head 3-40 or
3-56 (?) screws for covers and bottom plates? And the internal star lock
washers?Or whatever I am supposed to use?Seems to be a size not too popular
any more.
 Also,how to get the remaining screw shank out after the head is broken off?
And why does this happen? Cross threading by over zealous techs? 
 The world wonders.
