Only if you are trying to send code significantly faster than you know
how to.
Or if you don't know how to work an iambic keyer.

Yes it does happen.
It happens when either or both of the above occur.
This is most likely to happen when someone who thinks they are a CW
expert starts trying to use an iambic keyer, and doesn't realize that
for high speed CW you must always slow down slightly when starting to
use ANY new keyer.
