[PARC] Field Day - Release and Waiver

Attn:   All Field Day Participants and Volunteers

As required by our Park Permit for Field Day, all participants and 
volunteers are to sign the attached form releasing the City from any 
liability for personal injury or death in any way related to 
participating in the event.

Please print the attached, complete and drop off at the ARES table on 
your first visit.  Blank forms will also be available at the ARES table.
