Never seen that one -- may have been something used in house for a business. When I worked in Wales, we had 110 volt tools so workers would not steal them to use on their 220 volts at home. We also had light bulbs with left hand threads so they would not go home too. Us American's had transformer - bought state side and shipped with our household goods so we could run things with the 220 house wiring in the British homes. We would give these away when we came home, rather than ship them back. (Mostly passed them on to other Americans located there.)
Try smoke -- when I was a young teen I took this little fan down that had been sitting above Dad's workbench for years -- want to see how it would run, so I wired it up and plug it in -- it ran real fast for a few seconds and then the smoke poured out. (It was a 6 volt defroster fan from an old pre-WWII car or truck (or at least late 40's early 50's). I put it back up over the work bench and cleared out the smoke as best I could. (Basement Shop). Never told Dad, he never ask about it. It was a universal motor so would run on either 6 volts DC or 6 volts AC, but 120 volts was a little hard on it.

Run into something and though I would ask the collective brain for some help. A friend contributed a squirrel-cage blower to my shop but the plug is unlike anything I've seen. Looking down the bore, there is the usual round peg at the normal ground position (6 o'clock), the usual flat blade at 3 o'clock, and another round peg at 9 o'clock instead of a blade. There are no labels, ID info, etc. on the housing. European? DC?
Before I hook it up to a 120V line and release the magic smoke, I thought I would ask.
