the biggest reason for your woes is that most software people don't know 
poo about how to design systems properly.  There is more bullshit built 
into sites than one can safely imagine.  Because most coders and 
so-called "designers" are ignorant dullards with little to no knowledge 
of actual computer science, they are wholly inept at what they do in 
terms of being able to deliver a system the design of which is rational, 
functionally minimized, correct, and whose human interface provides for 
a satisfying experience.

For the most part it is a simple as that.

As I used to tell my clients: there is no substitute for knowing what 
you're doing.  Never more true than with computer systems.
