Seems to me that large construction failures .. like occurred in the
Boston Tunnel  are often to do  coruptions, where substandered matterials
like poor quality concrete are fraudulently substituted for the quality
matterials intended and paid for.

In the rural areas,  reputation is so important:  the word of mouth is
such that a company doing shoddy work, or using substandered materials
won't be in business very long.

Had a friend from Denver, Colorado tell me that when he had his home
re-shingled. They stripped everything down to the plywood sheeting. Most
of the 4 by 8 sheets of plywood were originally only nailed with one nail
in each corner..  just 4 nails to a sheet of plywood.  The contractor
doing the reshingling found some other "short cuts"  that needed rework, 
turned into a lot of additional cost for the homeowner.

The original carpenters and forman that over saw work  were likely  15 or
more years   beyond prosecution.   I think those of us that build for
ourselves tend to "over build", but if it looks like a Do it yourself
construction, the inspectors look much closer at us, than the
"professionals"  that know how to cover their shoddy work.
