Puffer fish have neurotoxins in them, cut the sack, which is in the visceral cavity, and a drop gets on the meat, the customer is dead before they can even taste the fish. It is a delicacy because the meat also contains euphorics, or "happy drugs". The japanese lost their national treasure comedian to puffer, or "fugu". Since then, they have outlawed it preparation and serving. However, for a chuck of yen, you can find underground fugu chefs.
