to start, nice work.  Second  how much does it weigh? Including the 
shipping crate. if one man can handle it on a dolly don't worry about a 
forklift. I am guessing it may be awkward if not just too heavy 
though.So laying it down flat and boxing it up  with the 1/2 plywood top 
and bottom and oak or pine board sides is a good idea. then figure on 
some boards to raise it up for a forklift 3" or  4" is good. I have 
received  items on a job site wrapped in plastic and the foam you 
mentioned. I think it is a great way to ship. It holds everything snug 
inside very nicely. I am not sure if you have delicate work, if the foam 
gets in and around your work how easy it can be pulled out I think there 
should be no problem though.
you could also try wood blocks inside the crate to hold the piece 
firmly.  Figure on at least 2" of space all around your piece when 
making the crate
