Ive done it by taking a disk approx 3 inches in diameter and about 1 inch 
thick or so.

Take a ball bearing about the size of the ball on the vise and drive it 
into the center of the disk  this will create the dished out side while 
keeping the other side flat, lastly drill or cut a hole in the disk.

Another way would be to cut/drill your hole, heat the entire washer up to a 
bright orange, then reassemble the vise with the hot washer in 
place.  Crank down on the vise handle and let the vise seat itself  into 
the washer.  It may take a couple of times but it will fit better.
