[TheForge] Tongs & power hammer - a safety issue?

In 25 years of blacksmithing and having two power hammers in the shop for
over 20 years I have never had this happen,  and I have done a lot of foolish
things.  Nor have I ever heard of it happening to anyone else.

Has anybody else ever heard of someone loosing a finger or mangling one in
some tong reins under a hammer?

just curious.
