Hello Bob;
Sounds like the basics are covered..a few addenda;
Use discomfort as the standard and stay a little below it.
Always lift with your palm facing upwards, not down.
Keep the sore part moving as much as possible during recovery...aids in 
the transport of healing fluid ( synovial) as there is little 
vascularization in tendons.
Drop down in hammer weight till comfortable and slowly work your way 
back up as it heals.
Avoid steroids ( cortizone) injections...they cause  tendon 
deterioration in the long run.
Don't push it...the more scar tissue you build up, the easier it is to 
re-injure it..gets chronic easily.
Once the elbow calms down, slowly rebuild strength by  resting your 
forearm on a table or knee  and curl your wrist up wards, with the palm 
down..gradually ad weight in small increments. That rubberband exercise 
is good too.
Print out these instructions in red ink and rub them repeatedly on the 
afflicted part.
