Normal 'good practice' will suffice; that's the reason for the 
protection diodes.

Don't bother; if they're already present that should be adequate.  They 
carry only the regulator error current, and then only if the sense 
circuit opens or at least goes high-R.

would be fine.  Alpha and 'everybody else' make something similar.

Since this was to the TE reflector I wasn't certain that your app 
involved RF.  The comments you received regarding decoupling are 
appropriate.  Do evaluate any added capacitance for its effect on the 
supply.  The twisted pair load lines and shielded twisted pair sense 
lines will help the RFI situation somewhat.  Even STP load cable might 
help.  Those styles are available in a range of gauges up to at least 12.
