Well Dan, the whole time I was in Europe, prior to the
proliferation of CB around the world, the amateur 
community always used "my  name is" - an occasional "handle"
for those familiar with American slang familiarity.  So
I always attribute that term to the influence from CB.
"Copy" is a very old term - not sure if it originated in
Amateur, military or commercial service.  I have only
been in radio (swl and amateur) for about 50 years
so someone active before me perhaps can answer that.
I suppose that the more a term is heard, the more it
will come into common usage.  Much like the incorrect
usage of some words on tv news - lack of verbs in
descriptions and incorrect endings to some words.  
No wonder the English (U.S. version) is so difficult
for foreigners to learn.
