Here's a list of upcoming exam sessions in Indiana:

All that's involved is the 35 question written exam plus the $12 test
fee (which covers postage and such). Unlike most things with the
Federal government the Amateur Radio license itself is absolutely free.
Here's a list of what you would need to bring to a test session:
* Two (2) forms of identification (one must be a photo ID).
* Your FRN number or Social Security Number.
* Cash for test fees.
* A blue or black ink pen (for filling out forms).
* A pencil (for taking the exams).
* A calculator (if desired, for taking the exams -
non-programmable only).
> What are the frequencies that you can transmit on?
Amateur Radio Bands (all new licensees):
50-54 MHz, 144-148 MHz, 420-450 MHz, 902-928 MHz
Lesser used bands (all new licensees):
1240-1300 MHz, 2300-2310 MHz, 2390-2450 MHz, 3300-3500 MHz,
5650-5925 MHz, 10.0-10.5 GHz, 24-24.5 GHz, 47-47.2 GHz, 75.5-76 GHz,
119.98-120.02 GHz, 142-149 GHz, 241-250 GHz, all above 300 GHz.
Nearly all the ham bands above 420 MHz are shared with the government;
primarily military (RADAR, etc) and science (deep space, weather, etc).
Almost never an issue but there are some added restrictions near
designated areas of the country. Nothing in Indiana yet as far as I
remember.

"Now You're Talking" has always been pretty good. Radio Shack used to
sell it but I couldn't find it in their online catalog. You should be
able to find it at some book stores. But, you can also order it from
ARRL for about $20.
4th Edition covers tests before July 1 2003.
5th Edition covers tests starting July 1 2003:
