Regarding the coverage of DCA tower
If any airport has indigenous crash/rescue, those units would be 
equipped with tower and ground control freq's
Comm coverage from those ATC facilities was to extend past airport 
boundaries so as to provide comms with those units
I don't remember what design distances were
Many accidents occurred off airport but in the vicinity

Now a days this function may be implemented by a "patch' channel to a 
TRS system
