If the passenger wasn't wearing a seatbelt the impact could be from a
forehead, air bags are supposed to work for a restrained person.

OK. The area of impact looked localized, as if it had been made by a
forehead. A windshield costs about as much as the first fifteen seconds in
the ER, so it's not a bad tradeoff.

Not really that uncommon.  Some air bags mounted on the top off the dash
will break the windshield- especially if there is a GPS, EZ-Pass or scanner
sitting on the dash.
