For a visual representation of the frequency use bycPennsylvania's DCNR, I
would point you to two maps I put together and shared with Radio Reference:

The VHF radio network was designed to primarily serve the Bureau of Forestry in
what is presently known as DCNR. Some other state agencies (i.e. DEP, Fish &
Boat Commission, Game Commission) have access to the system on a secondary
basis. I actually have a newer map listing a couple new simplex frequencies,
but those can be found in the database at Radio Reference. At one time, DCNR
and DEP were a single agency...after the split, DEP became a minor user of the
system as cell phones grew cheaper and more widespread.
The 159.345MHz freq you list is the repeater input for certain areas. The
151.400 freq is the corresponding output for those repeaters. 151.400 is also
used in simplex mode where possible. Many state parks will use 151.445 for
direct comms, but will switch to the nearest Forestry repeaters when needed.
Other state parks will use the repeaters for all communications (just depends
on their local needs/preferences). http://www.dcnr.state.pa.us
