Derek,
 From my perspective you can't be the judge.  A mgr has some responsibility 
to see to it that fraudulent cards don't go out but there's all sorts of 
circumstances.
I have, and I know others have, chased DX in pairs or groups with other 
buddies in other countries (sometimes they will 'get you in') and sent 
cards together to save money.  I never would work a new one for a friend - 
that was always a prize to lord over him about :-)  but I'm sure some would.
The SM/CT one sure does sound like a sneak....you might want to 'hold' that 
card for a while.  The second case doesn't sound unusual at all.
