Joe,

Ask General Manager Jim Walsh to be sure, but my understanding is that any
reasonable proof you have in hand is sufficient.  You can write Jim at

If you have been continuously licensed over those 50 years, there is a
second award that you would also qualify for:  The 50 Years Continuously
Licensed Award.  "This is issued to members whose calls appear in call books
for at least 50 consecutive years.  If licensed in both 1941 and 1946, the
war years are counted as licensed years." There's a $5 charge to cover to
cost of doing those 50 or so call book lookups.
