Here's the scoop from QCWA's Chapter Manual:
"QCWA Golden Certificates are offered free to members. There are two kinds:
"Golden Anniversary Certificates are issued upon application and proof that
the member was first licensed 50 years ago and is currently licensed.
"55th, 60th, 65th, 70th and 75th (and now 80th) Golden Anniversary
endorsements are issued to members who are eligible and who apply for them."
With regard to proof, a copy of your first license is sufficient. Also,
QCWA Inc. maintains a complete set of call-books and will do the lookup for
you.
Keep in mind that your first call may not appear in the call book until the
year following the year you were first licensed. In that event, QCWA goes
by the year first published, unless you can produce a copy of your first
license.
No chapter membership is required. Go for it!
