I operated the IOTA a few years back from Ocracoke Island.  It is part of the "Island Group" as they define it of NA-067, which also includes Hatteras and other islands of the Outer Banks.  Each Island is not denoted separately, but they are grouped together.

NA-112 is further South, but according to my directory specifically excludes Topsail Island.

I don't believe that the population or accessability by a bridge has anything to do with their definition.  Their definition of an Island requires a certain distance from the mainland and a certain size.  Since Topsail is specifically excluded from this group, I suspect it has been previously requested to be included and denied due to not having met the proper specifications.
