The Avery Labels are Avery 5167.  You can also print to plain paper as well.
I wrote an Excel macro to fill out the sheet.  You type in the info on 
the first label, and tell it how many labels you want to copy to, and it 
does the rest.  You can also clear all the labels.
I was going to add more bells and whistles, but I got lazy. However, I 
wrote the Macro in Micro$oft 360, so I don't know if it will work with 
older versions of Excel (don't remember how I saved it).  If anyone 
wants the macro, you're welcome to it.

However, I don't think I can email it, because macros can be viruses and 
do other mischievous things, so a lot of ISPs block macros.
