[PaQSO] Summary Sheet not Fillable

HI Guys,

I was able to fill in the "total package" summary sheet using Adobe Reader
(attached). However, I couldn't fill-in the "stand alone" summary sheet
using Adobe Reader.

I deleted the other pages in the "total package"... try to fill-in the
attached summary sheet... Yes.. Please download first and then opening it
with Adobe Reader.
