[PaQSO] Summary Sheet not Fillable

Is anyone able to fill in the standalone summary sheet - http://www.nittany-arc.net/pqppdf/15%20PAQP%20Summary%20Sheet.pdf ? I have tried three different computers all with Adobe Reader 11.0.12 and it does not allow any fields to be filled.  Reader pops up an error that the extended features are no longer available.  The summary sheet in the package is fillable.  I was not sure if the PA QSO folks want the whole 19 page package file sent.
