FWIW, "surveymonkey.com" is a legit cloud service for creating and
distributing surveys. I don't know anything about who actually made
the survey you received but the service itself is quite popular for
legitimate business purposes, sending surveys to customers, business
partners, employees, etc.
The HR department where I previously worked used it to poll employees
on how they thought the company was doing. I don't think they liked
the results they got back but they trusted it enough to use for internal
corporate business. You have to pay for that kind of use so people also
trust it enough to pay for.
