The really big VoIP companies will use things like SS7 and terminate DS3 or larger pipes into a suitable switching center.

The "little" VoIP might just be justing ISDN pri interfaces.

At one time Vonage used to receive calls at local exchanges, but outbound calls were put onto east or west coast gateways. I don't know if that is still the case.

In a case like that, Vonage was sending calls as if they came in another long distance carrier, or as if you had used a 1010xxx dial-around.

They never seemed to have call routing issues while I was a customer.

Another issue to keep in mind is that frequently alternate
VOIP companies (and others) negociate with individual
telcos & carriers to terminate thier traffic.  These negociations
identify NPA/NNXs desired by the alternative companies
and hence, may leave out more rural, remote locations
where little revenue is anticipated.  Subsequently, a major
carrier/company terminates traffic to certain exchanges
and blocks others...leading to confusion similiar to
what is being described...  Requesting the alternative
provider to negociate and open a particular NPA/NNX
is the right approach...but, tedious...  Like Luigi in
Chicago says..."it's not personal..it's business"!
