Korean Air Lines used to have a horrible safety record. This was because of Asian societies deference to more senior and older authority figures. The concept of "crew resource management" was not followed, resulting in many accidents where the Captain of the plane made incorrect judgements or flat out mistakes and the other crew members would not correct him because that would make him "lose face". It took a lot of deaths and, if I remember correctly, some bans by businesses from flying on KAL before KAL finally realized they had to do it. Today KAL's safety record is comparable to industry standard.
I have no doubt that the Captain screwed up on KAL 902 and the rest of the crew did not want to call the Captain out on his mistake. They "blamed" it on malfunctioning equipment. Yea, right. Even if one piece of equipment failed there were multiple pieces of other navigation equipment on that Boeing 707 that would have showed them that they were on the wrong track.
