Easy enough to grind down, in a pinch, but the three prong outlets have 
the blades offset, where-as, the plug in his pic, the blades were 
centered.  I went through my piles of cords, as well, and checked home 
improvement stores, no joy.  OLD extension cord ends were centered, as 
they had no ground, but the diameter may be off.

Ebay, for an old old unused cord-end, or just just the right connector, 
maybe?

Also, although not the exact cord, what about a two prong female 
extension cord end, but flat, rather than round?  I used them a lot in 
the late 80's - early 90's, just to get things running.  Sometimes I had 
to grind the edges down, or cut the corners off with wire cutters in a 
pinch.  Here's an example, should be able to get one at a local store, a 
thrift shop, at worst:
