[Milsurplus] B-24 radio log sheet 1944

So i bought this 'aircraft radio log page' off the Epay. Herewith the contents painstakingly rendered with my Toshitba computer. The form has no Army form number;
i suppose it is local base produced. I have tried to capture the original format but have not rendered the *'s and lines used across the page as dividers. I had thought
maybe i could reproduce the form for you to use on your own missions, but the form is way too dark, about paper bag darkness, and the acidic effect in the paper is
making it chip and split. The paper quality at best was about what i would call, "children's penmanship pad". I would like to eventually still reproduce the form, but
this would require a 1940s font typewriter - i donated my two nice ones years ago, which i slightly regret, but not greatly - or a 1940s typewriter font for the word
process program.
I invite your comments and interpretations. B-24-J were produced from Aug. 1943. The aircraft SN i do not know how to interpret; i looked at a list of Army aircraft
serials, and the only 5562 says "Lend Lease to Russia", but that seems real unlikely considering the date of the log. Also the log has to be for in the USA, i think, given
both its survival and the shortness of the second flight. I am guessing, very much guessing, that it was a training aircraft. QAL = "I will land at / Will you land at?"
and altho i have not come up with an airfield "SR", i am again, guessing that is Smyrna, Tennessee airfield, which trained on 4-motor planes, B-17 and B-24.
The letters in ( ) i think are the ground station's response. I don't have any idea what the numbers represent except the first is obviously the time sent. I was at first
surprised by the brevity of the transmission, but that's the point of Q codes and numeric codes. I wonder if radio watch meant having to stay at the HF receiver
listening to static. That would make one wish for squelch. But maybe on training or supply flights one could tune around for music between position reports
I recall reading in "Time To Get Out" that planes after a bombing
mission were to report back; this i suppose to keep track of the plane and its result in case the plane didn't make it back.
