I have also seen a military very experienced CW op in action and running
burst code at over 80 wpm on a typewriter while reading from a book and
discussing the text with me. He explained that the "brain" is not involved -
in the sense that he did not have to think about it...It was not distracting
to him at all....The CW came in his ears and his fingers moved appropriately
on the typewriter just using the basic hearing and movement pathway...  I am
not sure that this would be at all distracting while driving. It is a rare
skill.

He explained to me that on the long hours on duty, they would get so bored
because their brain was not able to "think" and follow at that speed so that
they often read a book or talked about something to keep from going crazy.

Reminds me of  Don C. Wallace: W6AM who I knew as a kid  in  Palos  Verdes
California ...  
he  would also  drive,  talk and use a knee key at the   same  time   also.
Thinking about that  now  with the   warnings of  not  texting while 
driving  or talking on the phone
it leaves you in wonder...   I never heard of  him being in  an   accident 
though...  must have had  great
multi-concentration!
