Good evening David,

I believe "ICP" would be Inventory Control Point; "SUR" would be 
Surplus; "13-64" would be the Julian date (13=13 January) of 1964.

32-64, for example, would be 1 February 1964; 100-xx would be 10 April 
for non-leap years, or 9 April for leap years, etc. Usually Julian dates 
having only two digits are padded with leading zeros to provide 
uniformity for computer entry use but for manually scribbled notations, 
as found on your unit, use of only the two digits would not be unexpected.
