If the source PDF isn't locked, you can use Acrobat (NOT Acrobat Reader) or 
one of several other PDF editors to extract the individual pages.  Under 
the Document tab, select Extract.  In the next window change "From: [1[ To: 
[1] Of 5" to "From: [1[ To: [5] Of 5" and click the "Extract Pages As Separate 
Files" box.  Click OK.  Select where to save them and by default the five 
pages will be saved as (original file name) -1 through -5.
