Ray,
Our contact person at Frederick County Health Dept. is the Director of 
Public Health Preparedness. The MD State Health Dept. was probably the 
agency that issued the kits about 3-4 years ago. The kits are in large 
(23"x18"x14") black plastic "Specialty Cases" along with a Buddy Pole 
antenna bag.
