at the trop of the page is a green link to the, "CARET Membership Application: [On-line form]"

Also, send a phone in the subject use your name & call sign, the CALL is more important.

Last, all photos should be relative close-ups and single color light backgrounds and single color shirts. The shirt should be either dark or light since I'll be putting your call on the photo & multicolor or medium brightness shirts are difficult to find a color for the text that shows well.

Check your photo to make sure your head can be clearly delineated from the background, if not try a different background.
