They are available at any electrical supply house, made by Square D and
others.  Cost should be about $25 or so (The one I installed 6 years ago
cost $19).  If you feel comfortable working inside a live panel, they are
simple to install.  If you don't feel comfortable, get an electrician to do
it.  Takes about 10 minutes.
