I think this is a misprint in the instruction manual.  It is more like 16v
and 200 mils.  There are many threads on this in the Icom archives.

While the newer Icoms will key the newer amps like the Alpha Power 87/89/91
and more than likely the Commander/QRO/Ten-Tec, you are asking for trouble
when keying direct the older amps like the L4-B/L-7/SB-220 etc that take
massive amounts of current for their keying relays.  The key is to check the
specs of the keying relay of your amp and be governed accordingly.  However
don't allow 2 amps as the current carrying capacity of the Icom 756 series
for sure.
