Both!

The transformer provides the voltage / current that
run the finals.  If the finals are not drawing current
then the transformer is drawing much less current. 
When the finals are "driven" they draw current from
the transformer which, in turn, draws current from the
AC mains.

The total power drawn from the AC mains is basically
the same between 120 VAC and 240 VAC.  It is just the
fact that the current is halved when on the higher
voltage.
