Dan -
The old one really needs to come out. Otherwise the new one sits above
the level of the knob rim.
One way is to drill a small hole in the center of the old insert, thread
in a sheet metal screw and pull. Some recommend drilling a hole from
the back of the knob and pushing the insert out from the back. One sure
thing is that you do NOT pry the old insert out using the rim of the
knob as a fulcrum! :-)
