There are no extra holes required. The cover just slips over the switch.
There is a slit that goes over the screw that holds on one of the 6.3V
lamps. There is a hole in the back of the cover that allows you to loosen
that screw to get the cover off.
It seems to have been added just a bit before they started using ceramics in
place of the BBODs.
In the version that has the resistor to the FCU rather then the capacitor,
the bare wire going to the VFO can be sitting at 200V or so. The version
uses the capacitor is quite safe, so I'm not sure why they bothered with the
cover, except maybe sensitivity - at higher frequencies, if you just wave
your hand around that switch, the tuning changes.
