Very interesting Glen.  A friend of mine with a KWM-2, had the exact same
problem which seemed to centered on the 6AZ8 which controls the relay keying
relay.

He did the same thing you did except cleaning the pins of the tube.
Wiggling and push in and out.  He eventually replaced the tube and the
problem was solved.  I forwarded your msg to him and he is now going to take
a very close look at the pulled tube pins as you did.
