I helped a guy out about two years ago on the knob, none left. But I 
have used a similar sized (it fits inside the main tuning knob) unit
threaded to the shaft. I used 1/8" brazing rod for that fix.

The plate is pretty heavy material as I recall about 1/8" in thickness
and steel. The 4 sides bend down but not much about 1/4". You could make
it with the proper tools.
