[Hallicrafters] HT-41 Amp

I have just been given an HT-41 Amplifier.  Having never used or had an
amplifier before I am uncertain as to how to connect it to my Icom IC
756 Pro 2.  The ham friend who gave the HT-41 to me had been using it
with a Kenwood 520 and had an automatic antenna relay switch.  He wanted
to keep the switch but told me I need one.
 
Any information or drawings as to how to connect the two and put the
HT-41 into use would be greatly appreciated.
