Mark, not sure I understand your line of questioning.  Especially as it pertains to number 3.  It is my understanding that you need the "i-Telex device" regardless of whether or not you have a Telex capable Teletype or just a plain ole M28KSR like I do.  Unless of course I'm misunderstanding what you mean by the "i-Telex device."  I have my 'plain ole' M28KSR connected to a set of boards I got from Henning and crew several years ago.  That set of boards  is in turn connected to the Internet and provides for the "i" in i-Telex.  The set of boards is a power supply board that provides the loop current for the TTY and the correct voltage for all the logic circuitry in the "device."  You have to provide a transformer (I just used the one they recommended) to provide power from your AC outlet to the device.

If you have a Telex compatible TTY that has the complete "call control unit" the you order one type of the i-Telex device, or if you have a non-Telex TTY such as the M28KSR, then you order the other type.  The only difference in the two is the board that connects to the loop.  You also have to provide an external relay for motor control when using the non-Telex device.  On a Telex ready machine the motor control is part of the call control unit.

Russ and Henning did I get it about right?

I have had no issues using my M28KSR vs having a Telex ready TTY.  In fact I find it to be a relief not to have to be concerned with using a call control unit.  One less point of failure to have to be concerned with.

Now if your wondering how do I make a call if I don't have a call control unit,  it's simple (Henning and crew have take care of that), I just hit the "BREAK" key, that wakes the system up, turns on the motor, types "NO?" on the TTY, and I just type in the number that would have been "dialed" on a call control unit equipped machine.  Easy peasy!
